QUARTERLY REPORT
2007-2008 School Year

Jacob XXXXX
D.O.B.: 09/XX/97
Age: 10
Grade: 5

Grading Key:             Effort & Skills
4 - Advanced               E - Excellent                  U - Unsatisfactory
3 - Proficient                G - Good                        X - Not Applicable
2 - Approaching          S - Satisfactory
1 - Below                     N - Needs to Improve

Days of Instruction:  63

Math: Jacob has been using Saxon Math this past quarter and reviewed for his standardized test.  Grade: 2 (S)

Reading: Lord of the Flies, The Pearl Grade: 4 (E)

Spelling: Jacob excels in spelling, reads constantly above and beyond assignments. His vocabulary and comprehension skills are closer to that of an 8th grader than that of a 5th grader. Grade: 4(E)

Writing, Grammar & Language Arts: Jacob has tactile-defensiveness issues when it comes to touching paper, and therefore the act of writing. Via dictation he continues to expound very intelligently on any topic he is assigned. He is very eloquent, articulate and his grammar skills are beyond grade level. Grade: 4 (E)

Health & Safety: Traffic safety and bike safety (though he cannot ride a two wheeler as of yet, he needs to be acquainted with bike safety) Grade: 4(E)

History: U.S. Government and World History (Barron's Review) Grade: 3(G)

Science:  This last quarter Jacob has continued the Barron's Review Course Series "Let's Review" Chemistry:The Physical Setting, Biology: The Living Environment, and Physics: The Physical Setting Grade: 3(G)

Art: Jacob continues to despise drawing. I have continued relying upon online games featuring things of an artistic nature to engage his interest and, again, it only does so on a minimal basis. This is an area where his lack of interest seems to have a direct correlation to his ability. His fine motor skills (as well as in the physical act of writing too) and his tactile defensiveness to paper makes this process of improving his artistic ability very difficult.  Grade: N (1)

Music: Jacob continues to practice piano - beginner level course. Grade: 2 (S)

Physical Education: Jacob is getting more active with the help of some video games that feature physical activity. He has been exercising on the Total Gym and going for walks with his family. Grade: 2 (S)

Typing & Computer: Jacob's typing is accurate and fast for his grade level, however he still needs to improve his technique, as I'd like to see him rely more on the "home base" position for fingers and refrain from bad typing habits.  Grade:  3 (S)

Teacher Comments - 4th Quarter
Jacob had a very good year, all things considered, We are looking forward to homeschooling for the 6th grade. He is a sweetie pie who excels in the homeschool setting. I just LOVE HIM!
